Free checklist for support coordinators

Get the bad days into the reassessment

A one page checklist for participants whose disability or condition isn't always visible. Use it in the weeks before a plan reassessment, so the evidence shows uneven days, hidden effort and the supports holding things together, not just how someone presents on the day.

We'll reply with the PDF. It's free, and there's nothing to sign up for.

One good day can end up as the whole picture

A planner can only work with the evidence in front of them. For someone whose function changes from day to day, a reassessment that lands on a good day can miss most of what their week looks like.

Since 1 October 2026, funding for social, civic and community participation supports and for capacity building daily activities has been reduced in new and reassessed plans. That doesn't mean every participant's funding drops by the same amount. From 1 February 2027, unspent funds won't carry over to a new plan, and since 27 August 2026, only participants, plan nominees and child representatives can ask for a plan reassessment.

Six prompts on one page

Each prompt comes from one lens of the UNSEEN framework. Start as soon as the plan end date is known, and ask the participant (and, with their consent, someone who knows them well) to keep a short record of good, usual and bad days for two weeks.

  • Uneven days

How often bad days happen, how long they last, what sets them off, and what a bad day stops them doing.

  • Net effort

Tasks that get done at a cost: the time they take, the rest needed afterwards, and what gets dropped to make room.

  • Supports removed

Who and what holds things together (family, prompts, routines, workers), and what happens when that support isn't there.

  • Every setting

Where they cope on the surface and pay for it later, and help to describe their usual and worst days at the assessment.

  • Evidence measures

Recent reports that describe current function, not only the diagnosis, with changes in numbers, such as “three or four days a week”.

  • Name the need

Each support being asked about, linked to the functional impact it addresses.

Made for participants with psychosocial disability, autism, ADHD, chronic illness, chronic pain or brain injury, and anyone whose function changes from day to day.

Something to share with participants

Getting ready for your plan reassessment is a free large print guide for participants and families. It walks through what to do before, during and after the reassessment, with a two week record they can print.
